to eligible individuals seeking to remain in the community rather than
live in a long-term care facility through the use of specific home

AGING AND DISABILITIES RESOURCE CONNECTION (ADRC)
The ADRC provides outreach, assessments and case management services to seniors and individuals with disabilities. Knowledge staff are available to provide information for such programs as adult day care, support groups, transportation, home care, respite, senior centers and home delivered meals to name a few. If necessary, a case mangers can complete a home visit and along with the resident and family identify the needs and create a service plan to meet those needs. AIDS COMMUNITY CARE ALTERNATIVES PROGRAM (ACCAP)
The AIDS Community Care Alternatives Program (ACCAP) program provides community-based services for individuals who are in need of long-term institutional care. The purpose of ACCAP is to help eligible individuals remain in the community rather than be cared for in a nursing facility or a hospital setting. For more information click here...
ASSISTED LIVING PROGRAM (ALP)
The Assisted Living Program is an in-home program that provides a package of assisted living services, including nursing and personal care, by and agency licensed by the Department of Health and Senior Services. ASSISTED LIVING RESIDENCE (ALR)
An Assisted Living Residence is a residential facility licensed by the Department of Health and Senior Services to provide apartment-style living and congregate dining and assure assisted living services are available when needed. Apartment units offer, at a minimum, one unfurnished room, a private bathroom, a kitchenette and a lockable door on the unit entrance. For more information click here...

The Jersey Assistance for Community Caregiving (JACC) is a program that provides in-home services for seniors at risk of placement in a nursing home. JACC is designed to supplement the assistance and individual receives from his/her natural support network. OUTREACH
Outreach staff conduct home visits to any Atlantic County resident
60 years of age or older in need of home delivered meals, personalized
attention for completing applications or explaining programs and benefits
