ASSISTED LIVING PROGRAM (ALP) and ASSISTED LIVING RESIDENCE (ALR)

ASSISTED LIVING PROGRAM (ALP)

The Assisted Living Program is an in-home program that provides a package of assisted living services, including nursing and personal care, by and agency licensed by the Department of Health and Senior Services. This program is available only to individuals residing in publicly subsidized housing. An individual continues to be responsible for rent, food, and other household expenses.

Services Available

The ALP provider shall provide or arrange for the provision of assistance with personal care, nursing, pharmaceutical, dietary and social work services to meet the individual needs of each resident. Services also include homemaker, chore, supervision of and assistance with self-administration of medication and administration of medications, therapeutic social and recreational activities, intermittent skilled nursing services, ongoing assessment and health monitoring, and transportation as specified in the plan of care. In addition to the ALP services, all participants receive all New Jersey Title XIX Medicaid services except nursing facility, personal care assistant services, Medicaid hospice, and adult day health services. The care plan is developed and agreed upon by the Assisted Living Program, participant and/or family member, and a care manager.

Paying for Services

Since participants are responsible for rent, food, and other household expenses, there is no cost associated with the Assisted Living Program.

ASSISTED LIVING RESIDENCE (ALR)

An Assisted Living Residence is a residential facility licensed by the Department of Health and Senior Services to provide apartment-style living and congregate dining and assure assisted living services are available when needed. Apartment units offer, at a minimum, one unfurnished room, a private bathroom, a kitchenette and a lockable door on the unit entrance.

Services Available

The ALR provider shall provide or arrange for the provision of assistance with personal care, nursing, pharmaceutical, and dietary services to meet the individual needs of each resident.
Assisted Living offers all New Jersey Title XIX Medicaid services except nursing facility, personal care assistant services, Medicaid hospice, and adult day health services. The care plan is developed and agreed upon by the Assisted Living Residence, participant and/or family member, and a care manager.

Paying for Services

All participants are responsible to pay a monthly room and board cost of $663.55. Additionally, participants with an income above the SSI amount of $753.05 per month may have a cost share liability for services to offset Medicaid costs.

You may be eligible for ALR or ALP if you are:

  • Age 65 or older, OR
  • From ages 21 through age 64 and determined disabled by the Social Security Administration Medicaid's Disability Review Section, AND
  • Assessed by the Department of Health and Senior Services' Long Term Care Field Office as in need of a nursing facility level of care, AND
  • Have a monthly income that is not more than $1,809 per month and resources of $2,000 or less or are currently receiving Supplemental Security Income (SSI) or New Jersey Care.
