The ADRC provides outreach, assessments and case management services to seniors and individuals with disabilities. Knowledge staff are available to provide information for such programs as adult day care, support groups, transportation, home care, respite, senior centers and home delivered meals to name a few. If necessary, a case mangers can complete a home visit and along with the resident and family identify the needs and create a service plan to meet those needs. The services available through the ADRC are designed to help residents to remain living in the a community setting.
