JERSEY ASSISTANCE FOR COMMUNITY CAREGIVING (JACC)

The Jersey Assistance for Community Caregiving (JACC) is a program that provides in-home services for seniors at risk of placement in a nursing home. JACC is designed to supplement the assistance and individual receives from his/her natural support network. Qualified individuals may have the opportunity to hire their own eligible family, friends, or neighbors to provide the care they need.

  • Home-Based Supportive Care
  • Transportation

JACC participants work together with a care manager to develop a Plan of Care to best meet their needs. The care manager then works with the participant to ensure that services are delivered as planned.

You may be eligible for JACC if you are:

  • Age 60 or older, AND
  • Require a nursing facility level of care but wish to remain at home, AND
  • Are not financially eligible for Medicaid or Medicaid Waiver Services, AND
  • Have resources at or below $40,000 for an individual, or $60,000 for a couple, AND
  • Reside in a home or an apartment that is not a licensed residence.
