AND DISABLED (CCPED) |
The Community Care Program for the Elderly and Disabled is a program
that provides in-home services to seniors at risk of nursing home placement.
Services Available
- Home Health Aide
- Home Health Nurse
- Adult Day Care
- Non-emergency Transportation
- Homemaker
- In-home Physical, Speech, and Occupational Therapies
Every CCPED participant works with a care manager to develop a Plan
of Care to best meet his or her needs. The care manager than works with
the participant to ensure that services are delivered as planned.
You may be eligible for CCPED if you are:
- At least 65 years old, or between 21 and 64 and determined
disabled by the Social Security Administration or by the Division of
Medical Assistance and Health Services, AND
- Residing in a home or apartment that is not a licensed residence
- Assessed by the Department of Health and Senior Services' Long Term
Care Field Office as in need of a nursing facility level of care, AND
- Have a monthly income that exceeds the Supplemental Security Income
(SSI) standard but is not more than $1,809 per month, AND
- Have resources
of $2,000 or less, AND
- Are eligible for Medicare or have health
insurance, which includes hospital and physician coverage
