CAREGIVER ASSISTANCE PROGRAM (CAP)

The Caregiver Assistance Program (CAP) is a program that provides in-home services to seniors at risk of nursing home placement. CAP is designed to supplement the assistance and individual receives from his/her natural support network. Qualified individuals may have the opportunity to hire their own eligible family, friends, or neighbors to provide the care they need.

  • In-home Physical, Speech, and Occupational Therapies
  • Home Health Aide
  • Home Health Nurse
  • Participants also receive a select package of Medicaid State Plan Services, such as physician visits, pharmaceuticals, and hospital coverage.

Every CAP participant works with a care manager to develop a Plan of Care to best meet his or her needs. The care manager than works with the participant to ensure that services are delivered as planned.

You may be eligible for CAP if you are:

  • At least 65 years old, or between 21 and 64 and determined disabled by the Social Security Administration or by the Division of Medical Assistance and Health Services, AND
  • Residing in a home or apartment that is not a licensed residence
  • Assessed by the Department of Health and Senior Services' Long Term Care Field Office as in need of a nursing facility level of care, AND
  • Have a monthly income that is not more than $1,809 per month and resources of $2,000 or less or are currently receiving Supplemental Security Income (SSI) or New Jersey Care.
